Job Description
A Care Manager is responsible for assessing and coordinating the personalised care of individuals, particularly those dealing with health challenges or social care issues. They collaborate with healthcare providers, social workers, and other professionals to create tailored care plans. Care Managers act as advocates, guiding clients through healthcare systems, ensuring access to needed services, and offering emotional support. They continuously monitor progress, revise care plans, and connect clients with community resources for holistic support, emphasising effective communication, empathy, and organisational skills.
Requirement
Education: A bachelor’s degree in social work, nursing, psychology, or a related field. A master’s degree will be an added advantage
Experience: A practical experience in health and social care
Knowledge: Strong understanding of healthcare systems, social services, and resources available for individuals in need.
Communication Skills: Excellent communication and interpersonal skills to effectively interact with clients, healthcare providers, and social workers.
